Registered Nurse (RN)
Midwest Labor is currently recruiting for a direct hire position for a Part-time Registered Nurse.
Position Summary:
The Registered Nurse (RN) is responsible for overseeing nursing-related services, ensuring compliance with Wisconsin DHS regulations, and supporting high-quality, person-centered care for clients receiving services through Adult Family Homes (AFH) and Personal Care Assistant (PCA) programs. The RN provides clinical oversight, staff guidance, and care coordination in accordance with ForwardHealth and Medicaid requirements.
Clinical Oversight & Care Coordination
• Conduct initial and ongoing nursing assessments as required
• Develop, review, and update individualized care plans
• Monitor client health status and respond to changes in condition
• Coordinate care with physicians, case managers, and other healthcare providers
• Review and monitor medication-related supports (non-administration unless authorized)
Compliance & Documentation
• Ensure compliance with Wisconsin DHS, ForwardHealth, and Medicaid regulations
• Review and maintain accuracy of client binders and clinical documentation
• Participate in audits, surveys, and corrective action planning
• Ensure emergency care plans (ECPs) are current and implemented
• Complete incident reviews and provide follow-up as required
Staff Support & Training
• Provide clinical guidance and supervision to caregivers and PCA staff
• Conduct staff training on client-specific care needs and nursing-related tasks
• Support competency evaluations and ongoing education
• Promote infection control, safety, and best practices
Quality Assurance & Program Development
• Participate in quality improvement initiatives
• Identify areas for improvement in care delivery and documentation
• Support policy development and implementation
• Maintain professional standards and ethical practice
Qualifications
• Current Wisconsin Registered Nurse (RN) license in good standing
• Minimum 1–2 years of nursing experience (home health, long-term care, or AFH preferred)
• Knowledge of ForwardHealth, Medicaid, AFH, and PCA regulations preferred
• Strong documentation, organizational, and communication skills
• Ability to work independently and collaboratively
• Proficiency with basic computer systems and electronic documentation
Physical & Work Requirements
• Ability to travel between client homes as needed
• Ability to perform assessments, observe care, and review documentation
• Ability to lift or assist as required by client care activities (as applicable)

• Maintain strict confidentiality in accordance with HIPAA and agency policies
• Adhere to ethical nursing standards and agency procedures
• Maintain professional boundaries at all times
